All requirements for Healthy Living and Physical Activity are being worked “in advance.” All activity and tracking between August 1, 2023 and July 31, 2024 will go toward determining the level attained effective October 1, 2024.

PREVENTATIVE CARE

Employees must complete any THREE preventive care visits throughout the plan year.

Examples:

Well Woman, Well Man, Mammogram, Pap Smear, Colonoscopy, Dental Exam #1, Dental Exam #2, Vision Exam, Vaccinations such as Flu, Shingles or TDAP.

NEW HIRES OR EMPLOYEES NEW TO COVERAGE DUE TO A QUALIFYING EVENT

BONUS Entries (not required for discount) – HEALTHY ENGAGEMENT – No minimum requirements
This Will Include Activities in the Community that Promote Health, Volunteerism etc.

Examples: Hutch Rec Bike to Work, Blue Cross Walk-at-Work Day, United Way Community Workday, Walk with a Doc, Volunteer Coaching, Girl/Boy Scout Leader.

PREVENTATIVE CARE

You are required to show proof of visit to earn credit. Please ensure that all personal information such as diagnosis, medications or etc. is not on your submission.

All submissions for preventative care must have proof uploaded within 30 days of the service.

EMPLOYEE PRORATION GUIDELINES DUE TO A QUALIFYING EVENT
Requirements for Healthy Living will be pro-rated based on Month of Hire:

If Hired Between August 1, 2023 through January 31, 2024:
(1st and 2nd Periods)
Full Requirements for Healthy Living must be met.
If Hired Between February 1, 2024 through April 30, 2024:
(3rd Period)
Lunch and Learns: 4
Newsletters: 1 per full Month of Employment
Preventive Care: waived until the new plan year starts August 1st but highly encouraged.
If Hired Between May 1, 2024 through June 30, 2024:
(4th Period)
Lunch and Learns: 2
Newsletters: 1 per full Month of Employment
Preventive Care: waived until the new plan year starts August 1st but highly encouraged.
If Hired Between July 1, 2024 through July 30, 2024:
All requirements are waived

EDUCATION

Employees will be required to sign in at all sessions if they are live.

Certificates will be provided to attendees which will need to be uploaded to the portal for final credit.

Proof of attendance must be uploaded within 30 days.

Educational Sessions will be added throughout the year. However, don’t wait until the last minute!

Recorded sessions will be added throughout the year and posted as soon as possible. Completion of a quiz with an 80% is required to receive credit.

Live Lunch and Learns will require participation. DOUBLE CREDIT will be given when attending in person. Participants who spend excessive time on their phones or talking to others will be asked to leave and attend another session.
